To attend the daycare the following papers are needed.

   * Current vaccination papers; your pet must be up to date with Distemper and Bordetella (kennel cough) shots. We also strongly suggest deworming and a flea program of some type.

   * Application Form: Available online and at the daycare. This form can be completed and either emailed to us at info@playfulpaws.net, or printed and brought in on your first visit


   * Waiver Form: Available online and at the daycare. As this form requires a signature and initials, please bring in the hard copy on your first visit.


In addition to the forms and vaccination requirements, all dogs must be spayed or neutered.

We reserve the right to end attendance at the day care due to aggression, excessive mounting or because it is not a good fit.
